Insurance & Rates



Below is a list of some of the major health insurance providers that we accept.    

Please note that insurance coverage varies depending on your particular policy and that not all insurance plans include chiropractic coverage.  Additionally, some plans may only provide partial coverage of our services.  We would be happy to help you check on your insurance coverage, simply call our office and have your insurance information in hand (408) 448-4445.

How does out-of-network billing work?

For plans with out-of-network chiropractic benefits, we will be able to submit a claim on your behalf.  

We will collect the fee at the time service is rendered (the cash rates will apply).  Then we can submit the claim to your insurance company and any reimbursement will be paid directly to you.  

Alternatively, we can provide you with a superbill, which you can submit directly to your insurance company for reimbursement.

Cash & Out-of-Network Rates 

$150 Initial Evaluation – includes consultation, examination, and treatment 
$80 Treatment Visit – includes chiropractic manipulation, soft tissue therapy and/or corrective exercises

$25 Laser Therapy – treatment for 1 region/round. Each additional region/round of laser is $10
$5 Kinesiology Taping Add-On  – includes any kinesiology tape application during a treatment session
$25 Kinesiology Taping Only Visit - includes discussion & application of area being taped (no treatment)

Insurance – rates will vary depending on your particular policy and your treatment program.  Please review our insurance information.  If you have insurance, but choose not to use your insurance for our services, then the above cash rates will apply.
Cancellation Policy

Our goal is to provide the highest quality of care to our community. We make every attempt to be as flexible as possible with our schedules to accommodate our patients’ busy lives, and we do understand that unanticipated events will occur. In our commitment to providing exceptional service to all of our patients, and out of consideration for our clincians’ time, we have adopted the following policies:
  • In the event that you need to reschedule or cancel your appointment, we request that you please give our office at least 24 hours notice. This will make the appointment time available to another patient. You may call our office (408) 448-4445 or email us
  • Appointments missed or cancelled without providing at least 24 hours prior notice are subject to a $25 missed appointment fee.
  • If you arrive late for your appointment, you will be seen as soon as possible, however your session may be shortened to the remainder of your original scheduled appointment time. If you arrive more than 10 minutes late, you may be asked to reschedule if we are unable to accommodate such a shift in our schedule.
  • If more than two visits are missed or cancelled at the last minute, advanced payment or credit card information will be requested at the time you schedule your next appointment.
Please Note: Appointment reminders are provided as a courtesy.  The reminder emails and texts are a program feature of our electronic system and may be subject to technical failure. Please do not rely solely on these reminders for your appointment.  If you do not receive your reminder email or text, the cancellation policy will still remain in effect.
